Apply – Masjid Accreditation

Please prepare the following documents before filling out the application form:

1 Request Letter from the Mosque/Cultural Center Administrator/Imam
2 Photocopy of SEC Registration, Article of Incorporation & By-Laws
3 List of Officers & Members
4 Barangay Certification as to the existence of the Mosque/Cultural Center
5 Mayor's Permit, if any
6 Pictures of the Mosque/Cultural Center, Office Building, Structures and other related photos
7 Report of ocular inspection by NCMF-CAD, if necessary for validation
8 Duly accomplished application form & information sheet by authorized representative

Please ensure all documents are complete and up-to-date. Incomplete applications will not be processed.

Download the official Masjid Accreditation Form. Fill it out completely and submit it to the NCMF NCR office along with the required documents.
