Apply for accreditation as a Sheikh / Imam with the National Commission on Muslim Filipinos – National Capital Region

List of Requirements

Please prepare the following documents before filling out the application form:

  • 1 Endorsement Letter from the NCMF Regional Office
  • 2 Barangay Clearance
  • 3 NBI or Police Clearance
  • 4 Medical Certificate (issued within the last 2 months)
  • 5 Two (2) recent 2x2 ID Photos

Please ensure all documents are complete and up-to-date. Incomplete applications will not be processed.
